All Seneca employees and students that access the Medical Centre for care.

Policy
1. Access to care
- The Medical Centre is operated by Seneca for use by students, employees and contingent workers. Visitors to campus may also be treated for minor medical needs.
- Access to services within the Medical Centre requires a Seneca OneCard and proof of insurance. Proof of insurance includes: Ontario Health Insurance Plan (OHIP) card, other provincial health insurance cards or an international health insurance certificate. Validation of international health insurance may be used to confirm a patient’s association with Seneca.
- OneCard and proof of insurance are non-transferable and must not be used for any improper or unlawful purposes, such as false representation as a registered Seneca student or employee. Health insurance fraud may have legal repercussions.
2. Service and confidentiality obligations
- All Medical Centre employees sign a mandatory Medical Centre - Employee Agent Confidentiality Agreement to protect patient privacy (See Appendix A).
- The Medical Centre is bound by the Personal Health Information Protection Act (PHIPA), which outlines the terms for the collection, use and disclosure of personal health information.
- Medical Centre staff, by condition of employment, must adhere to PHIPA standards for safeguarding patient privacy and the confidentiality of personal information by:
- collecting information only as necessary for patient care
- safeguarding health records on file, sharing information with other health care providers only when required for patient care
- disclosing information to third-parties only when written consent is received from a patient or when legally required
- retaining and destroying records in accordance with government regulations.

3. Patient consent and access to health records and service
- A patient’s request for care from the Medical Centre implies consent for the collection, use and disclosure of personal health information for care-related purposes.
- Patients have the right to see and obtain copies of their health records, or to request a correction to a record that is inaccurate or incomplete, subject to PHIPA.
- Confidential health information distributed by email may only be provided if a patient provides signed electronic consent.
- At each visit, patients are required to provide their home address, phone number, email, date of birth and health insurance information to access medical services.
4. Uninsured services
- Patients are required to pay for medical services not covered by health insurance, including urgent care, notes, forms, travel consults and employment physicals.
